Start Date ASAP Pay Rate Call for Details Job Description & Requirements Case Management Nurse - Case Manager - Travel (CM RN) The Disease Management Registered Nurse in the Blended role provides disease management nursing services and care coordination to high risk, complex patients (both adults and pediatrics) in a geographical region. The RN provides education, counseling and on-demand information to patients who have enrolled in a condition specific program. The RN will provide program education and coordinate care needs with the patient's Primary Care Provider. Services are performed in person or telephonically from a work-at-home location and interventions are recorded in the Population Care Management (PCM) application.
The role of the case management nurse is to coordinate continuity of care for patients often as a liason between the patient’s family and healthcare organization, ensuring that the proper treatment is administered at the appropriate time to maximize health and well-being while also minimizing the need for hospitalization. The CM RN strives to promote self-managed care and the use of healthcare resources in the most cost-effective way possible, working with patients of all ages and conditions, but primarily focusing on a specific population, such as children, families, the elderly, HIV/AIDS, acute care, or other specialties. Work is administered in a variety of settings, including HMOs, community health organizations, long-term care facilities, behavioral health programs, rehabilitation centers, schools, and case management companies. Required Qualifications -Must live in Austin, Texas
-Registered nurse responsible and accountable for safe clinical practice, knowledgeable of customers, and regulatory expectations in the clinical area.
-Current TX RN license in good standing and a minimum of 3 years of clinical experience, preferably in a field environment.
-Entry level clinical telephonic and field experience may include basic knowledge of nurse call center procedures and/or company products.
-Pediatrics experience preferred.
-Good communication and telephone skills.
-Basic computer skills.
-For patient and provider facing, staff must have:
-current CPR certification
